What We Do
- Work with patients to improve their overall social health
- Complete an assessment of the person’s mental and social wellbeing
- Connect patients to applicable community resources
- Support and educates patients in accessing and navigating health and social systems
- Facilitate group education sessions
What We Do Not Do
- Legal matters, including legal advice or interpretation of legal orders that may be in place
- Psychological assessments
- WCB or other insurance casework
- Guardianship/Trustee applications or advice—these types of requests should always be referred to the Office of the Public Guardian and Trustee (OPGT)
