About The Grande Prairie PCN

The Grande Prairie Primary Care Network, in beautiful Grande Prairie, Alberta – a city rich in cultural and economic opportunities, and a great place to raise a family – was formed in 2010 to work with local primary care physicians in the delivery of primary healthcare services. The PCN provides innovative and collaborative initiatives that improve communication, increase coordination of services, and enhance medical clinic practices to better meet the needs of the community.

Position Summary

The Proactive Care Assistant will act as an administrative staff member that will be accessible to Grande Prairie Primary Care Network (PCN) member clinic teams. The Proactive Care Assistant is responsible for executing and building capacity for clinics to engage in panel identification, maintenance, and management initiatives in the PCN’s member clinics. The Proactive Care Assistant builds capacity in the clinic environment, supporting panel processes and related quality improvement initiatives. Engagement and relationship building will be a key to success in this role.

What you will do...
  • Support clinic teams to manage the care of the patient panel
  • Conduct panel identification and maintenance activities
  • Assist clinic teams with Electronic Medical Record (EMR) standardization
  • Identify patients for proactive care and establish condition specific registries and outreach lists
  • Scheduling appointments as required
  • Work with PCN Practice Facilitators and clinic teams to document and sustain clinic specific proactive care approaches.
  • Other duties as required
What you need to succeed...
  • Fluency in English (verbal and written)
  • Excellent communication and interpersonal skills
  • Proficiency in the use of computers and office equipment
  • Strong MS office skills (Word, Excel, Teams)
  • Demonstrated ability to use electronic correspondence/forms
  • Ability to efficiently use EMR software
  • Ability to function cooperatively in a team setting
  • Ability to maintain patient privacy and confidentiality
  • Ability to efficiently manage time and prioritize multiple tasks
  • Ability to understand and apply change management principles with a variety of stakeholders
  • Medical Office Assistant certificate preferred but not essential
  • A valid drivers license and access to a vehicle
  • High School diploma
  • Experience working in a family practice or other health care setting considered an asset
