Unattached Complex Care Clinic – New Service

Description of Service

The Unattached Complex Care Clinic supports patients in the Grande Prairie area who are being discharged from the Grande Prairie Regional Hospital and do not have a family physician. The Grande Prairie PCN and Alberta Health Services have collaborated to establish the Unattached Complex Care Clinic, which aims to help the transition of patients back to community care by temporarily managing medical needs to stabilize patients and decrease the risk of further hospital readmissions.

Service Providers

The Unattached Complex Care Clinic is supported by a multidisciplinary team of physicians, nurse practitioners, registered nurses, social workers, mental health therapists, registered dietitians, and exercise therapists.

18yrs or older
Chronic condition(s)
Unattached (no primary care provider)

Appointments are booked or requested by the clinical teams at the Grande Prairie Regional Hospital.
