Mental Health Therapy Information For Member Clinics

Description of Service

Our mental health team includes Masters-level psychotherapists who hold professional association and/or registration. Our team provides short-term counselling and treatment to individuals in need. Due to the short-term nature of our service, our mandate is for mild to moderate mental health concerns.

Who is Eligible?

Physician members of the GPPPCN can make referrals for individuals 14 years and older.

Exclusion Criteria

Our mental health service does not provide the following:

  • Couples therapy
  • Addiction treatment
  • Personality disorder treatment
  • Education and return-to-work assessment
  • Crisis intervention or treatment
  • Treatment of severe, persistent psychiatric diagnoses
Referral Process & Referral Requirements

A referral for mental health therapy can be made using the PCN Central Referral Form. It is helpful to include both a PHQ-9 measuring symptoms of depression and a GAD-7 measuring symptoms of anxiety. If patients have past psychiatric diagnoses and/or are on psychiatric medication, please include this information. Once referrals are received, they are reviewed by our mental health navigator who completes an intake assessment to confirm that the patient is appropriate for PCN services and will assist with navigating the patient to supports while they await their mental health therapy appointment.

What to Expect

Once you refer your patient to mental health therapy, we will send communication back to your office to either confirm that the referral has been accepted or to notify you that the referral has been declined and provide suggestions for alternative mental health supports.  Once the patient is connected to a therapist, you will receive progress updates back periodically as well as a discharge update upon completion of the service. How to Prepare Your Patient For The Appointment

No. There is no cost to the patient to access mental health therapy at the PCN.
