Dietitian Information For Member Clinics

Description of Service

Our team of registered dietitians provides free individual nutrition counselling, education and support for adults and children 14yrs or older. Our dietitians work with a patient to improve their quality of eating and nutrient intake to manage their health. They develop individualized nutrition plans considering the patient’s goals and values as well as their health history. The goal is to assist the patient with tools and skills to self-manage their nutrition and eating.

  • Celiac disease
  • Chronic disease management: hyperlipidemia, hypertension, diabetes, or pre-diabetes
  • Disordered eating
  • Emotional eating
  • Food allergies/intolerances
  • GERD
  • Gout
  • Inflammatory bowel disease (IBD)
  • Iron anemia or other nutrient deficiency
  • Irritable bowel syndrome (IBS)
  • Mindful eating & Food relationship
  • Post-bariatric surgery nutrition
  • Prenatal and postpartum nutrition
  • Weight management
  • Vegetarianism/veganism
  • Other non-acute nutritional concerns
Who is Eligible?

Physician members of the GPPCN can make referrals for individuals 14 years and older.

Exclusion Criteria

Our dietitian service does not provide the following:

  • Support for active eating disorders
  • Pediatric weight management
  • New Type 1 Diabetes patients or Gestational Diabetes
  • Children under 14yrs old
Referral Process & Referral Requirements

A referral for dietitian services can be made using the PCN Central Referral Form. It is helpful to include a patient medical history and medication list along with the referral.

What to Expect

Once the patient is connected to a dietitian at the PCN, you will receive progress updates back periodically as well as a discharge update upon completion of the service. If your referral does not meet our criteria, we will send communication back to your office to notify you that the referral has been declined and provide suggestions for alternative nutrition supports. 

How to Prepare Your Patient For The Appointment

Please discuss and inform your patient of their referral to see a dietitian at PCN and why the referral is being made. It is helpful to include a readiness screen with your patient to assess their conviction and confidence in making dietary changes.

FAQs

Is there a cost for dietitian services?
No. There is no cost to the patient to access a dietitian at the PCN.

Will you provide my patient with a meal plan?
No. Although we may work with a patient to provide specific macronutrient and/or micronutrient targets and teach a patient how to create their own meal plan, we do not supply generic meal plans.
