Nurse Case Manager

What We Do
  • Initial assessment & provide ongoing education regarding Pain Services and team
  • Clarify goals and treatment outcomes with the client/patient
    • Connects the client with the services that are best-suited to support them in achieving their goals in Pain Services
    • Provides options and recommendations based on assessment and learnings on what is important to each individual patient
    • Initiates and facilitates development of the treatment plan
  • Supports the client in navigating Pain Services
    • Connects with the patient at regular intervals to assess & offer supports
    • Validation of the person’s experience with pain
    • Facilitates Client engagement while in Pain Services
  • Communicates with referring providers regarding progress in Pain Services
    • Ensures a Discharge Summary and /or Flare-up plans are provided to Patient/Client & Physician upon discharge from Pain Services
What We Do Not Do
  • Deter program participants from their abilities to self-manage in Pain Services:
    • We do not prioritize the participant’s goals, we support the participant in identifying what is most important to them to work on in Pain Services.
    • We do not provide reminder calls for follow-up appointments that were scheduled with the agreement of the participant
