Team-Based Care

Description of Service

Team-Based Care Implementation is a key pillar of the Patient’s Medical Home, focusing on the embedded allied health professionals working with physicians and their clinic teams to provide enhanced comprehensive care that is custom to that physician’s patient panel.

The TBC member will serve patients directly through one-to-one or group appointments and manage a corresponding panel of patients. As well as supporting transformation work undertaken by the physicians, and implementation of the Patient’s Medical Home as it relates to the allied health professional’s clinical activity.

Current Team-Based Care

The Grande Prairie Primary Care Network has over 20 physicians that are utilizing embedded Team-Based Care members (LPNs and RNs) in their clinics including:

 

  • Family Practice Nurse (RN or LPN)
  • Chronic Disease Management Family Practice Nurse
  • Women’s Health Family Practice Nurse
  • Geriatric Family Practice Nurse
