Description of Program

The Transformation Team includes a Program Manager, Practice Facilitators, and a Nurse Educator who work together to support primary care transformation activity in our member physicians’ clinics. The Transformation Team facilitates practice level changes in primary care delivery in line with the Patient’s Medical Home model of care. The Transformation Team focuses on enhancing team-based care by supporting collaboration between physicians and PCN team members.
