About The Grande Prairie PCN

The Grande Prairie Primary Care Network, in beautiful Grande Prairie, Alberta – a city rich in cultural and economic opportunities, and a great place to raise a family – was formed in 2010 to work with local primary care physicians in the delivery of primary healthcare services. The PCN provides innovative and collaborative initiatives that improve communication, increase coordination of services, and enhance medical clinic practices to better meet the needs of the community.

Position Summary

The Social Worker’s primary function is to work with patients to determine, advocate for, and refer to appropriate community resources and programs. The Social worker will collaborate with Primary Care Physicians, their clinical staff and other members of the Primary Care Support Program and Persistent Pain Program to deliver support using a patient-centered approach.

What you will do...
  • Implements a comprehensive patient assessment process
  • Facilitates goal development with patients and works towards completion of identified goals
  • Works with patients to build capacity to self-manage their conditions and circumstances
  • Ensures that all relevant data and history is available and entered into the Electronic Medical Record (EMR) in accordance with practice standards and organizational policy
  • Communicates with team members regarding patients’ concerns, needs and progress as necessary
  • Participates in case consultation with clinical supervisor
  • Facilitates patient education sessions
  • Makes appropriate referrals to other services and resources
  • Collaborates with community agencies as required
  • Undertakes community visits related to patient needs
  • Maintains current knowledge and skills in social work practice
  • Maintains knowledge of and adherence to organizational policies and procedures
  • Adheres to Canadian Association of Social Workers’ code of ethics and guidelines for ethical practice
  • Maintains professional requirements and actively participates in ongoing professional development
  • Actively participates in program development and quality improvement activities
  • Offers training support for other team members and assists in their professional development
  • Acts as a resource to other PCN staff
What you need to succeed...
  • Registration with Alberta College of Social Workers
  • BSW, RSW or MSW, RSW
  • Minimum 2 years of community based front line experience
  • Experience in a health care setting considered an asset
  • Fluency in English language (written and verbal)
  • Demonstrated ability to empathize with and understand individual patient needs
  • Ability to deal effectively with the difficult behaviors sometimes exhibited by people in distress
  • Ability to assess and manage risk
  • Ability to function cooperatively with physicians and clinic staff in a multidisciplinary setting
  • Ability to build a good working relationship with patients, families and other healthcare professionals
  • Effective teaching and facilitation skills for clients, colleagues and students
  • Ability to make evidence-based decisions based on social work practice standards
  • Ability to monitor best practices, develop new skills and apply theory to practice
  • Ability to identify when input and collaboration with other professionals would enhance case management
  • Ability to utilize supervision appropriately
  • Ability to occasionally work outside of normal business hours
