About The Grande Prairie PCN

The Grande Prairie Primary Care Network, in beautiful Grande Prairie, Alberta – a city rich in cultural and economic opportunities, and a great place to raise a family – was formed in 2010 to work with local primary care physicians in the delivery of primary healthcare services. The PCN provides innovative and collaborative initiatives that improve communication, increase coordination of services, and enhance medical clinic practices to better meet the needs of the community.

Position Summary

The Registered Social Worker’s primary function is to work with patients to determine, advocate for, and attain appropriate community resources and programs. The Registered Social Worker will collaborate with referring physicians, their clinical staff and other members of the PCN Team to deliver support through a patient-centred approach. This is a 0.80 FTE permanent position with flexibility to be a 1.0 FTE.

What you will do...
  • Assess, plan, and evaluate a care plan for patient’s social needs
  • Provide solution-focused interventions and assist with practical matters, such as: Financial services (income options, form completions, budgeting, debt management); Medication coverage; Community Resources (career and employment counseling, caregiver support, disability supports); Guidance for available housing options
  • Advocate for patients in a health care setting
  • Utilize problem solving techniques in relation to patient care
  • Work with patients to build capacity to self-manage their conditions and circumstances
  • Ensure that all relevant data and history is available and entered into the EMR
  • Make appropriate referrals to other services and resources
  • Participates in case consultation
  • Collaborate with community agencies as required
  • Undertake community visits related to patient needs
  • Maintain current knowledge and skills in social work practice
  • Ensure that PCN policies and procedures align with social work practice standards
  • Maintain professional requirements and actively participate in ongoing professional development
  • Adhere to Canadian Association of Social Workers’ code of ethics and guidelines for ethical practice
  • Offer training support for other team members and assist in their professional development
What you need to succeed...
  • Registration with Alberta College of Social Workers
  • BSW
  • Minimum 2 years of community based front line experience
  • Experience in a health care setting considered an asset
  • Previous use of an electronic medical record system considered an asset
